Glaucoma is a group of eye conditions that damage the optic nerve—the connection that carries visual information from the eye to the brain.

The damage often develops slowly and without obvious symptoms. Treatment cannot usually restore vision already lost to glaucoma, but lowering eye pressure can help protect the vision that remains.

What causes glaucoma?

The front of the eye continuously produces a clear fluid called aqueous humor. The fluid normally drains through tissue located where the iris and cornea meet.

When drainage does not work properly, pressure inside the eye may increase. Higher eye pressure is an important risk factor for optic-nerve damage.

However, glaucoma is not defined by pressure alone. Some people develop glaucoma even when their measured eye pressure falls within a statistically typical range. This is called normal-tension glaucoma.

A complete diagnosis therefore considers the optic nerve, visual field, drainage angle, corneal thickness and eye pressure together.

Why glaucoma can go unnoticed

The most common forms of glaucoma usually develop gradually and without pain.

Early blind spots may be difficult to recognize because:

  • One eye may compensate for the other.
  • The brain may partially fill in missing information.
  • The changes may occur outside the center of vision.
  • Vision loss may progress slowly over several years.

A person may still read an eye chart clearly while having meaningful visual-field loss. Regular comprehensive eye examinations are important because symptoms alone cannot reliably detect early glaucoma.

Major types of glaucoma

Primary open-angle glaucoma

Primary open-angle glaucoma is the most common type. The eye’s drainage angle remains physically open, but fluid does not drain efficiently enough.

Optic-nerve damage and visual-field loss usually develop gradually.

Normal-tension glaucoma

Normal-tension glaucoma causes characteristic optic-nerve damage even though pressure measurements are not unusually high.

Treatment generally focuses on lowering eye pressure from the person’s individual baseline.

Angle-closure glaucoma

Angle-closure glaucoma occurs when the iris blocks the eye’s drainage angle. The closure may develop gradually or happen suddenly.

Sudden—or acute—angle closure can cause a rapid increase in eye pressure and is a medical emergency.

Secondary glaucoma

Secondary glaucoma develops because of another condition or exposure, such as:

  • Eye injury
  • Eye inflammation
  • Abnormal blood-vessel growth
  • Certain structural abnormalities
  • Some medications, including corticosteroids in susceptible people
  • Complications of another eye condition

Never stop a prescribed steroid or other medicine on your own. Ask the prescriber and eye-care professional how the medicine affects your individual risk.

Childhood glaucoma

Childhood glaucoma is uncommon and may be present at birth or develop later. Possible signs include cloudy-looking eyes, excessive tearing, light sensitivity or unusually enlarged eyes.

A child with possible glaucoma requires prompt specialist evaluation.

When glaucoma symptoms are an emergency

Seek emergency medical care immediately for sudden, intense eye pain—especially when accompanied by a red eye, blurred vision, nausea or vomiting.

These can be symptoms of acute angle-closure glaucoma. The National Eye Institute advises going to a doctor or emergency room immediately when these sudden symptoms occur.

Do not wait for a routine appointment or try to treat the symptoms at home.

Sudden flashes, many new floaters or a curtain-like shadow can indicate a different eye emergency, such as retinal detachment, and also require immediate evaluation.

Who is at increased risk?

Anyone can develop glaucoma. Risk may be higher with:

  • Increased eye pressure
  • Advancing age
  • A parent or sibling with glaucoma
  • A thinner central cornea
  • Previous eye injury or surgery
  • Certain eye conditions
  • Long-term or repeated corticosteroid exposure
  • Some ancestry and population patterns

In the United States, Black or African American adults have a higher risk of open-angle glaucoma and may develop it at younger ages. Hispanic and Latino adults have increased risk, particularly at older ages. Some Asian populations have higher rates of angle-closure or normal-tension glaucoma.

These are population-level patterns. They do not determine an individual person’s diagnosis or prognosis.

Ask an eye-care professional how often you should receive a comprehensive dilated examination based on your individual risk factors.

How glaucoma is diagnosed and monitored

No single test provides the complete answer. Glaucoma evaluation commonly combines several types of information.

Eye-pressure measurement

Tonometry measures pressure inside the eye. A pressure reading provides useful information, but one normal reading does not rule out glaucoma.

Pressure can vary during the day and between appointments.

Dilated optic-nerve examination

A dilated examination allows the clinician to evaluate the optic nerve and look for changes in its shape or remaining nerve tissue.

Photographs may be used to create a baseline for comparison.

Optical coherence tomography

Optical coherence tomography, commonly called OCT, produces detailed images of the optic nerve and retinal nerve-fiber layers.

Repeated scans can help identify structural change over time.

Visual-field testing

Visual-field testing maps how sensitive different areas of vision are to light. It can reveal functional blind spots that may not be noticeable during daily activities.

The test requires concentration, and results can vary. A clinician may repeat it to confirm a suspected change.

Gonioscopy

Gonioscopy uses a specialized lens to examine the eye’s drainage angle. It helps distinguish open-angle from angle-closure conditions.

Corneal-thickness measurement

Pachymetry measures the thickness of the cornea. Corneal thickness provides important context when the clinician interprets eye-pressure measurements and glaucoma risk.

Understanding visual-field loss

Glaucoma does not always create a simple narrowing that looks like a view through a tunnel.

Possible patterns include:

  • Small localized blind spots
  • Arcing areas of reduced sensitivity
  • Missing areas near the center of vision
  • Loss affecting the upper or lower field
  • Nasal-field loss
  • Broader peripheral constriction
  • Different patterns in each eye

A missing area may not appear black. It may seem blurred, faded, distorted or simply absent from awareness.

Visual-field loss can make it harder to notice:

  • Steps and curbs
  • Objects on the floor
  • Doorframes
  • People approaching from the side
  • Vehicles or pedestrians
  • Items on crowded shelves
  • The beginning or end of a line of text

Regular testing is necessary because a person may not notice progression independently.

How glaucoma is treated

Treatment is individualized according to the glaucoma type, severity, target pressure, rate of change, overall health and ability to use the treatment consistently.

Prescription eye drops

Glaucoma drops lower eye pressure by reducing fluid production, increasing fluid drainage or both.

Different medication classes work in different ways and can produce different side effects. Use drops exactly as prescribed and report problems to the prescriber.

Laser treatment

Laser procedures can improve fluid drainage or address an angle-closure risk.

Selective laser trabeculoplasty may be used as an initial treatment or together with medication for certain forms of open-angle glaucoma.

Laser peripheral iridotomy creates a small opening in the iris to treat or reduce the risk of certain angle-closure conditions.

Surgery

When medication and laser treatment are not sufficient or appropriate, surgery may create or improve a drainage pathway.

Procedures include:

  • Minimally invasive glaucoma surgery
  • Trabeculectomy
  • Glaucoma drainage implants

The best procedure depends on the glaucoma type and stage. According to the National Eye Institute, surgery cannot cure glaucoma or reverse established vision loss, but it can lower pressure and help protect remaining vision.

Treatment protects future vision

Glaucoma treatment may not make vision feel sharper. Its purpose is usually to slow or prevent additional optic-nerve damage.

Do not stop eye drops because your vision seems unchanged or because you are experiencing cost, comfort or administration problems. Contact the prescriber. Alternatives may include another medication, a combination drop, an administration aid, laser treatment or surgery.

Treatment lowers risk, but it does not guarantee that glaucoma will never progress. Continued monitoring allows the clinician to adjust the target pressure and treatment plan when necessary.

Using glaucoma eye drops

Follow the instructions provided for your particular medicine.

A general eye-drop routine may include:

  1. Wash your hands.
  2. Confirm the correct bottle and eye.
  3. Tilt your head back.
  4. Gently pull down the lower eyelid to form a pocket.
  5. Place one drop in the pocket without touching the bottle tip to your eye, eyelid, fingers or another surface.
  6. Close the eye gently without squeezing or repeated blinking.
  7. Follow your clinician’s instructions about applying light pressure near the inner corner of the closed eye.

If you use more than one type of eye drop, follow the timing supplied by your prescriber or pharmacist. The American Academy of Ophthalmology generally advises waiting approximately three to five minutes between different eye-drop medicines.

Ask a pharmacist or eye-care professional to watch your technique if you are uncertain whether the drop is reaching your eye.

Making treatment easier to manage

A reliable system should not depend on memory or bottle color alone.

Consider:

  • Phone alarms or talking reminders
  • A written or accessible digital dose record
  • Talking prescription labels
  • Large-print instructions
  • Distinct tactile markers on different bottles
  • A bottle-squeezing or positioning aid
  • Refill reminders
  • Linking drops to a consistent daily routine

Tactile markers should help locate a bottle, not replace confirmation from its official label.

Record a dose immediately after using it. An alarm can remind you that a dose is due, but it cannot confirm that the dose was taken.

If you miss a dose or cannot remember whether you used it, do not guess or automatically take another dose. Follow the medicine’s instructions or contact the pharmacist or prescriber.

Living safely with visual-field loss

Practice deliberate scanning

Move your eyes and head systematically to check areas that may fall outside your usable field. Scanning strategies should be matched to your particular field-loss pattern.

An orientation-and-mobility specialist can provide structured training for indoor travel, outdoor routes, street crossings and public transportation.

Improve lighting without creating glare

Use even lighting in hallways, stairways and room transitions. Avoid harsh pools of light, deep shadows and glare that can hide edges or obstacles.

Increase useful contrast

Contrast may help identify:

  • Stair edges
  • Doorframes
  • Light switches
  • Handrails
  • Frequently used objects
  • Bathroom fixtures

Any markings placed on stairs or floors should be securely installed and slip-resistant.

Keep walking areas predictable

Remove or secure:

  • Loose rugs
  • Electrical cords
  • Low furniture
  • Unmarked glass obstacles
  • Items left on stairs
  • Clutter in regular walking paths

Return commonly used objects to consistent locations.

Use mobility tools when appropriate

A long cane is not reserved for complete blindness. It can detect drop-offs, curbs and obstacles that fall outside the usable visual field.

An orientation-and-mobility specialist can determine whether a cane or another strategy would improve safety.

Driving and glaucoma

Visual-field loss can affect the ability to detect vehicles, pedestrians, cyclists and hazards when merging, turning or changing lanes.

Clear central acuity does not necessarily mean the entire visual field is adequate for driving. Requirements also vary by jurisdiction.

Discuss driving honestly with your eye-care professional. A visual-field assessment and specialized driving evaluation may help determine whether driving remains safe and legally permitted.

Do not depend on an informal self-test or another person’s reassurance.

Technology and daily-task strategies

Different tools address different problems.

Reading

Try line guides, increased spacing, text reflow, screen magnification or text-to-speech. A narrower visual field may make long lines harder to track even when individual letters remain clear.

Computer use

Helpful features may include:

  • Cursor enlargement
  • Pointer highlighting
  • Screen magnification with focus tracking
  • Simplified layouts
  • Keyboard navigation
  • Text-to-speech
  • Customized contrast

Finding objects

Reduce visual clutter and store important objects consistently. Use contrasting trays, tactile labels or smartphone visual-assistance tools.

Artificial-intelligence detection can be helpful but may be wrong. Do not rely on it for medication identification, street crossing or other safety-critical decisions.

  • What type and stage of glaucoma do I have in each eye?
  • What is my target eye pressure?
  • What was my pressure today?
  • How do my current visual-field and OCT results compare with my baseline?
  • Is the glaucoma stable, or is there evidence of progression?
  • Am I using my eye drops correctly?
  • What should I do if I miss a dose?
  • Which symptoms require immediate emergency care?
  • Could medication cost or side effects affect my treatment options?
  • Would low-vision rehabilitation help with my daily activities?
  • Should I receive an orientation-and-mobility evaluation?

Can glaucoma be cured?

There is currently no cure for glaucoma. Treatment and monitoring can lower eye pressure and help protect remaining vision.

Can lost vision be restored?
